Town of Milton Proof of Vaccine or Exemption

In accordance with Town policy C19-04, all Town employees and volunteers are required to provide proof of being fully vaccinated against COVID-19 or an approved exemption.

Please use this system to submit your proof of vaccination or proof of medical exemption. For vaccines requiring two doses for full vaccination status, only proof of the second dose is required.

Before uploading to this system, ensure you have:

An image or PDF of your vaccination records.

  • Those vaccinated in Ontario may download their vaccination receipt from the Government of Ontario website.
  • Those vaccinated outside Ontario must upload English documentation with:
    1. The dates the vaccines were administered
    2. The country/location in which the vaccines were administered


Proof of your qualification for an exemption. Exemptions will be granted only for medical grounds or other grounds protected by the Ontario Human Rights Code.

  • A representative from Human Resources will follow up with employees or volunteers submitting a request for vaccination exemptions after review of the submitted request/proof.
